H.O.R.S.E. Therapies of Northeast FL, Inc. accepts referrals from individuals, healthcare and/or educational providers. If you are interested in making a self-referral for you or a family member or if you are a provider working with someone you think would benefit from our services please click the link below to complete the on-line form to request a callback from our referral coordinator. Please take the time to review our website and familiarize yourself with our services and providers.
What to expect when you make a referral:
Once we receive the referral form, a H.O.R.S.E. Therapies of Northeast FL, Inc. Administrator will contact the identified contact person via email to set up a phone/video conference screening appointment to make sure we understand your needs and to discuss the services we provide, costs, etc.
Following the screening, a referral packet requesting more detailed information will be sent for completion. As soon as we get that back, your initial session will be scheduled to do a “meet and greet” at the farm. At this visit we will meet the client/family in the waiting area and the journey will begin from there. We will make introductions to the human and animal staff, review the safety rules and make sure everyone has enough information to make an informed decision about receiving services through H.O.R.S.E. Therapies of Northeast FL, Inc. At this session, when the client (and/or guardian) decides to continue, the provider will set up the next sessions and determine the perfect human and animal match for the particular situation.
Those receiving Equine- Assisted Psychotherapy (EAP) with a mental health professional will continue the intake process with a session or two to complete a clinical assessment and determine a treatment plan. These sessions are required for therapy, and once complete the experiential sessions with the horse can begin. It may take a few sessions to get your started, but once they begin the connection with our equine partners is worth the wait.
Those receiving Equine-Assisted Learning (EAL) with an Equine Assisted Specialist in Mental Health & Learning (ESMHL) will continue their intake process with their equine professional where you (client/guardian) will discuss in more detail your needs and your hopes for participating in this experiential education process. You may review pertinent medical/mental health/education history and current needs. It is highly likely that you will have an opportunity to meet your equine partner or at least get to know those who are options based on the client’s needs. By the end of this session, the client and ESMHL will have at least a beginning plan for where to begin the work over the next few weeks.
At any point in the referral process, referral sources and/or clients/guardians may contact us. As we are at the barn most of the time, email is the BEST way to reach us. Please email our team at referral@HORSEtherapies.org. If you feel the need to call, please contact Mrs. Candice Barnes or leave a message at our extension at the Jacksonville Equestrian Center
(904) 255-4228.
We look forward to seeing you at the farm!
